It is also based on a thorough knowledge of the complex process through which public policies are formulated, implemented, and modified.

In 1995, about 300 horizontally integrated hospital systems owned, leased, or managed about half of the nation’s hospitals (Dowling 1995). There are predictions that by the turn of the century only about 5 percent of all hospitals will remain independent (Savage et al. 1997). Horizontal integration is not limited to hospitals, however. Nursing homes, HMOs, physician groups, pharmacies, hospices, and others have also integrated horizontally. During this middle phase in the evolution from fragmentation to integration, the healthcare system’s landscape continued to include numerous independent organizations but there was a growing complement of horizontally linked groups of essentially similar organizations.
